FORMULARIO COORDINACIÓN PACIENTES EVALUACIÓN TELEMEDICINA

SE RUEGA CONFIRMAR DENTRO DE LAS 4 HORAS SIGUIENTES AL ENVÍO DE LOS DATOS DE TRANSFERENCIA. 

INSTRUCCIONES:

1. Servicio de TELEMEDICINA sin cambios en la modalidad.
2. El correo de coordinación puede demorar algunas horas en llegar. 
3. Adjunta los exámenes recientes del paciente al final del formulario.
4. Si el paciente está enfermo físicamente, primero debes consultar a un Médico Veterinario.
5. Revisa SIEMPRE la carpeta de spam por si tu email no ha llegado. Agrega contacto@etologiaclinica.cl a tus remitentes de confianza.
